PMR 31 - Form 2: Examination Appeal Form

Summary

The bases for appeal of an examination are appropriateness or correctness of item or items in written examinations; failure to follow proper examination procedures; or fraud or favoritism in the oral interview process. This form asks you to Identify which basis of appeal is alleged and describe the facts, information or circumstances upon which the appeal is based.  It also asked what remedy is sought.

There are three ways to submit the form:

  1. Submit electronically: Complete and submit the form online for County processing.
  2. Print and submit a completed form: Click the “Preview PDF” button at the bottom of the form to print it and submit using an alternative method.
  3. Print and complete by hand: Print a blank PDF version of the form, fill it out manually, and submit it using an alternative method.
